	RELEASE FORM The camper is in good health and has my permission to participate in camp activities. In the event of an emergency, I give permission to the medical personnel selected by the camp directors to render proper treatment. I understand that Summertime Sports LLC is not responsible for accidents occurring at camp resulting in medical, dental or other expenses. I also understand that Summertime Sports Soccer Academy is not owned or operated by Wesleyan or Yale Universities. I agree to hold harmless these institutions from any and all causes of action including those for injury or property damage.
